You can earn My Health Pays rewards from MHS Health Wisconsin when you complete healthy activities!

You will receive your My Health Pays Visa Prepaid Card when you earn your first reward from MHS Health. Each time you complete a qualifying healthy activity, we are notified and your reward dollars will be added to your existing card. It's that simple!

Earn Rewards

$25 - For completing a Health Risk Assessment (Medicaid SSI members only). Call us at 1-844-545-6326.
$25 - For having an HbA1c test as part of your diabetes care (once in the calendar year for members age 18-75).
$25 - For having a retinopathy screening (dilated eye exam) as part of your diabetes care (once in the calendar year for members age 18-75).
$25 - HealthCheck. Yearly well-child visit for children and young adults age 16 months to 21 years of age.
$10 - First 5 well-child visits. First 5 well child visits from birth to 15 months of age. $10 for each visit.
$25 - 6^th well child visit before age 15 months.
$25 - Lead screening before 2nd birthday. Limit one per child.
$25 - Childhood immunizations. Complete immunization series before 2nd birthday. Including all of the following:
- Dtap – 4 shots
- IPV – 3 shots
- MMR
- HiB – 3 shots
- HepB – 3 shots
- HebB Booster
- VZV
- PCV – 4 shots
$25 - Adolescent immunizations. Complete adolescent immunizations before 13th birthday. (meningococcal, Tdap, & HPV vaccine).
$25 - Annual child dental visit. Reward only applies to members in these counties: Kenosha, Milwaukee, Ozaukee, Racine, Washington, Waukesha.
$25 - For an annual breast cancer screening (One per calendar year; females age 50-74).
$25 - When you have a chlamydia test (once per calendar year, women age 16-24).
$25 - When you have a cervical cancer screening (women age 21-64).
$25* - When you notify us you are pregnant by submitting a completed Notification of Pregnancy (NOP) form.
$25 - For a postpartum doctor visit between 7-84 days after delivery.* Doctor's confirmation is needed.

*To be eligible for this reward, you must notify us you are pregnant by calling us at 1-888-713-6180 (TDD/TTY: 1-800-947-3529) or submitting a completed Notification of Pregnancy (NOP) form. Prenatal visit count begins after we are notified you are pregnant. Once you notify us you are pregnant, you will receive further instructions on how to submit proof of these doctor visits.
