How it Works

1. Do healthy activities. Go to checkups, get screenings, or other health activities that qualify.

2. Get your card. After your first activity, we mail you a prepaid My Health Pays Visa® card with your reward dollars. You can spend your rewards on everyday items at places like Walmart. This card may not be used to buy alcohol, tobacco, or firearms.

3. Keep earning. Each time you complete another activity, reward dollars are added to your card automatically.

Remember to keep your card even if your rewards balance is zero. When you earn more rewards, we’ll add it to your card. 

How to Earn Rewards

• $25 - (Medicaid SSI members only). Completion of a Health Risk Screening (HRS). Call us at 1-844-545-6326.
• $25 - For having an HbA1c test as part of your Diabetes care. Ages 18-75. One per calendar year.
• $25 - For having a Retinopathy screening (dilated eyeexam) as part of your Diabetes care. Ages 18-75. One per calendar year.
• $25 - Annual Well Child Visit. Ages 3 to 21. One per calendar year.
• $10 - Completion of up to 6 Well Child Visits. First 15 months. One reward per visit up to 6 visits.
• $10 - Completion of up to 2 Well Child Visits. Between 15 and 30 months. One reward per visit up to 2 visits.
• $25 - Child Lead Screening. Limit of 1 from birth to age 2.
• $25 - Childhood Immunizations Series (listed below).All must be completed before 2nd birthday. Including all of the following:

▸IPV - 3 shots
▸MMR
▸HiB - 3 shots
▸HepB - 3 shots
▸HepB Booster
▸VZV
▸PCV - 4 shots

• $25 - Adolescent Immunizations (meningococcal, Tdap, & HPV vaccine). Must be completed before 13th birthday.
• $25 - Child Annual Dental Visits. Reward only applies to members who receive dental benefits through MHS Health Wisconsin and live in Kenosha, Milwaukee, Ozaukee, Racine, Washington, or Waukesha counties. Ages 2-20. Once per year.
• $25 - Annual Breast Cancer Screening. Ages 40-74. One per calendar year.
• $25 - Annual Chlamydia Screening. Females ages 16-24. One per calendar year.
• $25 - Cervical Cancer Screening. Ages 21-64. One per calendar year.
• $25 - Pregnancy notification through a completed Notification of Pregnancy (NOP) form. This form should be sent to MHS Health early in pregnancy.
• $25 - Postpartum Doctor Visit* between 7 and 84 days after delivery. You must have notified MHS Health of your pregnancy to receive this reward.

*To be eligible for this reward, you must notify us you are pregnant by calling us at 1-888-713-6180 (TDD/TTY: 1-800-947-3529) or submitting a completed Notification of Pregnancy (NOP) form. Prenatal visit count begins after we are notified you are pregnant.  Once you notify us you are pregnant, you will receive further instructions on how to submit proof of these doctor visits.
